With several practice modalities to choose from, no matter where you are in the life of your practice, we have a model that can work for you.

Personalized Care Programs

Hybrid Model

The hybrid model fits physicians who have well established relationships with their patients but smaller patient panel sizes. In this model, physicians split time between personalized care program patients and traditional patients. Hybrid practices typically have fewer than 100 members.

Full-conversion Model

In the full-conversion model, physicians reduce their practice to personalized care program patients only and transition all traditional patients to other local physicians. This model is ideal for physicians who are considering retirement or those who place an absolute premium on work-life balance.

Market Segmented Model

Physicians operating a market segmented personalized care practice spend most of their time and attention on 200–600 personalized care patients and oversee an associate physician, nurse practitioner, or physician’s assistant who manages the care of the traditional patients. Affiliated physicians will often continue to consult with traditional patients but only when they present with complex issues.

Enhanced Services Model

The Longeviti Personalized Care and Enhanced Services Model shares similarities with the other models with one major exception. In the ‘Enhanced Services Model’ we work with you to develop your ideal practice structure as well as assist you in implementing a number of highly desirable cash based services designed to maximize revenue and dramatically increase patient satisfaction.

By way of example, practices that participate in the ‘Enhanced Services Model’ can anticipate making roughly $50,000 to $100,000 more than comparable market segmented practices.

Longeviti Enhanced Physical and Health Assessment

From the patients perspective, most annual health screenings consist of not much more than a nurse taking their vitals, recording their medications and asking a few general questions about their medical history. They may also be screened for additional risk factors such as smoking and diabetes. After this, they spend some time speaking with their physician during which time the patient is briefly examined. From there, the physician may order some follow-up tests including a blood panel or EKG with the suggestion that the patient lose a few pounds or take some supplements. Once this is over, the patient is generally on their own.

With fewer patients, physicians can devote more time to each patient going well beyond the basic level of care described above. Every year program members will participate in the Longeviti Enhanced Physical and Health Assessment. As part of this Program, patient’s receive comprehensive, advanced health screenings and diagnostic tests that have been shown to help detect issues earlier thereby providing actionable insights for you as their physician.

As part of the program, you and your patient will have the opportunity to discuss their health test results in detail. With guidance from you, you and your patient will work together to develop an individualized plan designed to improve their health and manage and track results.

Overall, the Program is intended to identify family risk factors and monitor and treat chronic illnesses so that your patients can meaningfully impact their health and live better, longer.
