The annual fee for individual memberships in Longeviti-affiliated practices is generally between $1,500 and $5,000, based on location. Please contact us to discuss pricing for your company.
No. They can choose to keep their current doctor while still taking advantage of the annual physical and health assessment while also having access to any enhanced services their Longeviti physician may provide.
Choosing a doctor is based on personal preference. Please have your employees contact us for details and contact information regarding Longeviti-affiliated practices in their area.
If your company doesn’t pay the full cost of membership for your employees, they can use pre-tax corporate plans to pay the fee. Following their annual wellness exam, employees can typically obtain reimbursement for the annual program fee from flexible spending accounts (FSAs), medical savings accounts (MSAs), health reimbursement accounts (HRAs), or health savings accounts (HSAs). Employees should be made aware that they cannot pay the program fee with their FSA debit or credit card.
Most Longeviti-affiliated practices accept insurance. Your employees’ annual fee pays for same or next day appointments, unhurried appointments that last as long as necessary, 24/7 access to your physician by phone or email and may also be used to cover preventive care medical services that insurance usually doesn’t cover (e.g., advanced diagnostic testing and screenings). Your employee’s Longeviti-affiliated physician will continue to bill their insurance and charge co-pays as he or she currently does for other medical services.
Longeviti-affiliated internal or family practice physicians will typically be able to treat your employees’ children (between the ages of 16 to 25) for routine office visits with no additional fee. This may vary from practice to practice. Please check with your Longeviti-affiliated physician to be certain. Standard fees, insurance copays, coinsurance or deductibles still apply.
In order to help your employees easily connect with your doctor and pay the annual fee, Longeviti has designed an online portal for communicating securely with your doctor, managing your payment profile and payment schedule, and contacting us with questions.
The process of setting up a Longeviti Employee Health Program for your company is quite simple. After you’ve connected with our team, you’ll execute a non-disclosure agreement and one-year corporate agreement, and Longeviti will enroll your company into the Program.
Once enrolled, simply submit a list of employees who are eligible for this new health benefit and we’ll help you distribute welcome communications that explain the program to those eligible employees.
